Attached is a separate document outlining a plan for restructuring my current full-time position as [job title] into an equally [more] productive telecommuting arrangement on [#] days each workweek.

If you want to return to work in a compressed workweek arrangement:

However, I propose a progressive approach to retaining my training and experience that can also lead to an increase in on-the-job [select: concentration; energy; loyalty; efficiency; creativity]. Attached is a separate document outlining a plan for restructuring my current full-time position as [job title] into a full-time and equally productive compressed workweek arrangement.
